By using LongevityFix's telehealth services, you ("Patient") consent to receiving medical evaluation, consultation, and treatment through electronic communications, including but not limited to video, audio, and messaging technologies. Please read this Telehealth Consent carefully before proceeding.

I. What is Telehealth?

Telehealth involves the use of electronic communications to enable healthcare providers to evaluate, diagnose, and treat patients at a distance. LongevityFix facilitates telehealth consultations between patients and independently licensed medical professionals. These communications may include interactive audio/video sessions, asynchronous messaging (store-and-forward technology), and secure electronic transmission of patient health information.

II. Patient Acknowledgments

By consenting to telehealth services through LongevityFix, you acknowledge and agree to the following:

  • You understand that telehealth consultations are conducted by independent, licensed medical professionals and not by LongevityFix directly.
  • You understand that a valid prescription may only be issued following a legitimate provider-patient relationship and a good-faith medical evaluation.
  • You understand that telehealth services may not be appropriate for all medical conditions, and the provider may refer you to in-person care when clinically necessary.
  • You understand that technical difficulties may interrupt or prevent telehealth services, and you consent to alternative communication methods if needed.
  • You confirm that you are located in a U.S. state or territory where LongevityFix services are available at the time of your consultation.
  • You are at least 18 years of age.
  • You understand that no prescription is guaranteed, and that all prescribing decisions rest solely with the licensed provider.

III. Validity of Prescription

LongevityFix and its affiliated medical providers comply with all applicable federal and state telemedicine laws and regulations. Prescriptions are only issued following a thorough review of your submitted health information and, where required, a live consultation with a licensed provider. Prescription medications will not be dispensed prior to the provision of care by a licensed medical professional and the issuance of a valid prescription.

All prescriptions are dispensed through independent, licensed third-party compounding pharmacies. LongevityFix does not operate as a pharmacy and does not dispense medications directly.

IV. Privacy and HIPAA Compliance

LongevityFix is committed to protecting your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and all other applicable federal and state privacy laws. All information you provide — including personal, medical, and financial data — is transmitted using Secure Socket Layer (SSL) encryption technology.

Your health information may be shared with affiliated licensed medical providers, pharmacies, and laboratories solely for the purpose of delivering your care. We will not sell or share your health information for marketing purposes without your explicit consent. V. Risks and Limitations of Telehealth

As with any medical service, telehealth carries certain risks and limitations, including:

  • Information transmitted may be insufficient for a provider to make a complete clinical assessment.
  • Delays in evaluation and treatment may occur due to technical failures.
  • Security protocols may fail, resulting in a breach of confidentiality.
  • A lack of direct physical examination may limit the provider's ability to diagnose certain conditions accurately.

You have the right to withdraw this consent at any time by contacting us at support@longevityfix.com. Withdrawal of consent will not affect any treatment already provided.

VI. State Availability

LongevityFix telehealth services are available to patients located in eligible U.S. states and territories. Services may not be available in all jurisdictions. The availability of specific medications, treatments, and services may vary by state based on applicable laws and regulations. You will be informed if services are not available in your location.
